Small groups generally consist of firms with 2-50 employees, although some states allow self-employed people - - so called “groups of one” – to purchase in the small group market. Operating under state law, fully insured coverage is subject to state benefit mandates.
Small group coverage is offered on a guaranteed issue basis - - that is, medical underwriting is not used to determine whether a small firm will be offered coverage. Small group premiums are determined by state rating rules, which set the degree of required “rate compression” based on the demographic and actuarial characteristics of the group, as well as actual or predicted health status.
Premiums are fully adjusted for non-health related demographic factors such as age and health status or claims experience. Thus, while the health status of a firm’s employees or dependents cannot be used to determine whether coverage is issued, it can be used to set rates in most states, at least to a degree.
Large Groups are typically those with more than 50 eligible employees, although the number may vary depending on state law. Large group health insurance contracts, unlike small group health insurance contracts, do not have to be offered on a guaranteed-issue basis; so a health insurance company reserves the right to reject an entire large employer group based on its claims history or COBRA participants.
Large group health insurance is medically underwritten at the time of purchase, with rates based on employee participation and prior claims experience. The health insurance company bases annual premium changes for large employer groups primarily on the claims experience of the group in the past years, as well as any overall increases in the cost of providing health insurance coverage.
